We have two ways for you to submit a volunteer application:

Adobe Acrobat Version of Application Form

Please download, print, and fill out a Volunteer Application Form in full and drop it off or fax it to the office.

Microsoft Word Version of Application Form

Fill out the shaded areas in Volunteer Application Form. Once you have completed the Volunteer Application Form save the file as your_name.doc. Attach the saved file and email to info@hscpc.com . A copy will be printed for you to sign during your first interview.
